HOW TO REGISTER FOR CHRONIC MEDICATION
  • If you are diagnosed with a chronic condition, you will be required to register with our Chronic Disease Management Programme via our call centre in order to receive your chronic medication.
  • Chronic medication may be collected from designated pharmacies nationwide and is subject to pre-authorisation.
  • You may download the Chronic Application Form here or you may contact us on 010 141 3053 and request that the application form be faxed or emailed to you.

SUBMITTING YOUR APPLICATION FORM
  • Fill in the application form in black ink using block letters. Please print clearly.
  • Answer all medical questions truthfully and include as much detail as possible.
  • Please make sure that the principal member signs and dates any changes.
  • Fax the completed and signed form to 086 597 2400 or email it to admin@acumenhealth.co.za  
  • Please attach copies of each applicant’s identity document. We also accept valid passports in respect of foreign nationals and birth certificates for children.
ACUMEN HEALTH PROCESS TO BE FOLLOWED AFTER WE RECEIVE YOUR APPLICATION FORM
  • If any details are missing or if we need any more information for underwriting purposes, we shall contact you.
  • We will activate your membership and send you a notification of confirmation via SMS within 24 hours which will consist of your Acumen Health membership number and other important contact information.
  • You will be notified when to expect your Acumen Health Welcome Pack via SMS.
